Last week I set up two bird feeders on one black iron pole which I bought from the Seattle Audubon store, just like billions of other people do. I bought the extra long length for the second hook, so the two hooks converge for a few inches with a very narrow space between, until the bottoms merge at the crotch of the pole.

I was horrified to come home yesterday and find a dead juvenile chickadee stuck in the crotch of the pole! Somehow its head got wedged in the teeny opening. I called Audubon; they said they hadn't heard of this happening before and they're going to check into it. Meanwhile I have taped up the section where the poles converge up to where the opening widens.
Just thought other double feeder owners should know about this....
